Doctors Urgent
Care Clinic-Based Health Care Option offers affordable, clinic-based
care to individual and families including:
Unlimited office visits.
No charge
for any service we can provide in the clinic. (visit

Preventive
medical care. (Yearly physical exam, chest
x-ray, EKG, labs)
Counseling regarding healthy lifestyles.
Cost of less than $3.00 per day for an individual and
less than $5.00 per day for family coverage.
Care provided by physicians, nurse practitioners and physician assistants.
Adobe® Reader® 6.0 is free software that lets you view and print (PDF) files on a variety of hardware and operating system platforms. Adobe reader is required to view and print the Clinic-Based Health Care Option application form.
*Hospitalization,
outpatient testing, specialists, and prescriptions are not covered.
** Although
we see children for illness, we are not pediatricians. Therefore,
we do not give immunizations and we insist that you have an established
pediatrician.

Doctors Urgent Care reserves the right to cancel contract with a
30 (thirty) day notice.
